Cancellation Policy: We understand that life happens, and you may need to cancel or reschedule your appointment.
- Please provide at least 48 business hours’ notice for cancellations
- Cancellations within 24 hours will incur a penalty fee equivalent to 50% of the services booked
- Same day cancellations or a no show, will result in a charge for the full amount of the services booked.
Late Arrivals:
- Please arrive on time for your appointment. If you arrive more than 10 minutes late, we may need to adjust your service or reschedule, as a courtesy to the next client.
- For services that cannot be shortened, late arrivals will result in a rescheduling fee.
Child Policy: Our facility is not a safe space for children due to sharps, chemicals & delicate equipment.
- For safety & insurance purposes, no children under the age of 13 are permitted
Payments:
- Payment is due at time of service. Payments accepted are cash, all major credit cards, or Apple Pay.
- HSA/FSA cards accepted with exceptions
- For packages or prepaid treatments, payment is required in full before the first session
Refunds:
- All sales on treatments, products, services and sales are final
- If you are unsatisfied with a service or experience adverse effects, please contact us within 48 hours of your appointment to address concerns & recommend solutions
Consent Forms:
- All clients are required to complete and sign consent forms before their treatment. These forms ensure you are informed about the procedure, potential risks, and aftercare.
- Consent forms must be updated annually or if there is a change in your medical history since last visit
Telehealth Medical Screening:
- Certain services require a medical clearance by our nurse practitioner or physician assistant to be performed before receiving and will be done at the time of your appointment.
- If you would like to perform exam within the 72 hours before your appointment, please contact us to schedule
- For medical weight loss programs, this consult is required every 4-6 weeks to monitor your progress effectively
- These telehealth screenings are mandatory for all new clients, and must be updated annually or if there is a change in your medical history
